Comments:
An unannounced Subsidy Health and Safety Inspection (SHSI) was conducted from 10:30 am - 2:30 pm with the owner. A licensing inspection was also conducted on this date (see that report for details). Five staff records were reviewed for documentation of the mandatory VA Preservice Training and current first aid and CPR certification. Three enrolled children have diagnosed food allergies and records were reviewed for allergy action plans. Based on review of 13 children's records, the facility failed to have a written allergy action care plan for each child diagnosed with a food allergy, to include instructions from a physician regarding the steps to take in the event of an allergic reaction.
Evidence: Child B and Child C did not have documentation of a written allergy action plan, with parent and physician signature, for a diagnosed food allergy.
Plan of Correction:
Children B and C are no longer at our school. We have reviewed files to ensure our regular students? files
are in compliance.

Description:
Based on a review of five staff files, and an interview with staff, it was determined that not all staff have current certification in first aid and cardiopulmonary resuscitation (CPR). Evidence: Staff #3 (date of hire: 2/5/2019) and staff #5 (date of hire: 11/7/2016) did not have documentation of CPR certification on file.
Plan of Correction:
Staff #3 has taken her CPR certification. Staff #5 is no longer employed by the school. All other staff members have current CPR certification.
