Comments:
An unannounced monitoring inspection took place today, October 24, 2019, beginning at 2:30 PM and ending at 5:00 PM. There were 17 children in care today with three staff providing care and supervision. There were no medications onsite during this inspection. All areas that are used for care were inspected, as were five children's records and all staff records, at a separate site, on September 10, 2019. The inspector looked at first aid and emergency supplies, drill documentation, and required health and fire inspections. The children were observed during the following activities: homework time, snack time, outside time, and watching a presentation from a special guest. There were discussions about children's records, staff records, and posting requirements. The violations were discussed at the conclusion of the inspection. Based on record review, the center failed to ensure that the licensed or registered child welfare agency or family day system does not have a central registry finding within 30 days of employment or volunteer service.
Evidence:
1. Staff 4 had a documented hire date of 12/23/2018 and a documented central registry check of 03/01/2019.
Plan of Correction:
"For future, we will ensure all staff documents are completed within a timely manner and before staff are working with students."

Description:
Based on record review and discussion with staff, the facility failed to ensure that no person shall be hired for compensated employment prior to the facility obtaining an employment eligibility letter upon the completion of a fingerprint background check as required by the Code of Virginia.
Evidence:
1. Staff 1 had a documented hire date of 08/05/2019 and a documented eligibility date of 08/07/2019. 2. Staff 4 had a documented hire date of 12/23/2018 and a documented eligibility date of 01/10/2019. 3. Staff 1 confirmed that Staff 4 and Staff 1 did not have documented employment eligibility to work prior to working for compensation.
Plan of Correction:
"For future, we will ensure all staff documents are completed within the correct hire and eligibility date."
