Comments:
A monitoring inspection was initiated on 12/13/2023 at approximately 7:55am and concluded on 12/13/2023 at approximately 9:20am. There were a total of 27 children with 6 staff members present. Children were observed during lining up for the bathroom, engaged in group activities and participating in circle time. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 6 child records and 4 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on review of staff files, the center did not obtain the results of a criminal history record information check or a search of the child abuse and neglect registry from any state in which the individual has resided in the preceding five years.
Evidence: There were no results of a criminal history record information check or a search of the child abuse and neglect registry from any state in which Staff #3 resided in the preceding five years.
Plan of Correction:
Center will obtain a criminal history record information check and a search of the child abuse and neglect registry from any state in which the staff resided in the preceding five years.
