Comments:
A monitoring inspection was initiated on 5/24/2024 at approximately 11:50am and concluded on 5/24/2024 at approximately 12:45pm. There were a total of 22 children with 2 staff members present. Children were observed socializing after school, lining up to go outside, and and outdoor play time. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 2 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on review of children's records, the center did not obtain the work phone number of each parent who has custody.
Evidence: There was no work phone number for a parent of Child #1.
Plan of Correction:
