Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews, and a virtual tour of the program. A monitoring inspection was initiated on 9/20/2021 and concluded on 9/20/2021. The director was contacted by telephone and a virtual inspection was conducted. There were 14 children present, ranging in ages from 3 years to 4 years with 4 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records and 2 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on review of two staff files, the facility failed to have documentation that each staff had orientation training by the end of their first day of assuming job responsibilities. Staff 2 (first day of employment 8/31/2021) did not have documentation of orientation.
Plan of Correction:
All staff will receive orientation by the end of the first day of assuming job responsibilities and it will be documented.

Description:
Based on review of two staff records and interview, the facility did not obtain an out of state sex offender check for each staff prior to hire for each state the individual has resided in the past five years. Evidence: Staff 1 (date of hire 7/19/2021) indicated on the sworn disclosure statement that she had lived outside the state of Virginia in the past five years. There was no documentation of an out of state sex offender check for Staff 1, and the director stated the facility had not done it yet.
Plan of Correction:
Center director will conduct a sex offender search for Staff A.
