Comments:
An unannounced monitoring inspection was conducted from 11:50 am to 12:50 pm. At the time of entrance, 5 preschool to school aged children were in care with the provider. The record sample size consisted of 3 children's records and the provider's and household member's records. The children in care were observed engaging with the provider, participating in free play, and eating lunch. There have been no injuries since the last inspection and no medications are being administered at this time. Areas of noncompliance are noted in the violation narrative and were discussed with the provider during the exit meeting at the conclusion of the inspection.
Based on observation it was determined the provider did not have the required emergency phone numbers posted close to the phone. Evidence: 1. 911, local police, fire, emergency medical responders, poison control, and emergency backup care person's numbers were not posted. 2. Provider confirmed these numbers were not posted during this inspection.

Provider will post the required emergency numbers by the phone.

Description:
Based on record review and interview it was determined the children's records did not contain all of the required information. Evidence: 1. The record for child #2 did not contain the medical insurance policy name and number, 1 of the 2 required emergency contacts, first date of attendance, and proof of identity. 2. The record for child #3 did not contain the medical insurance policy name and number, and documentation of the annual review of the record by the parent. 3. Provider confirmed that this information was not in the records during this inspection.
