Comments:
An unannounced monitoring inspection was conducted on 4/18/24. At the time of entrance, 3 children totaling 8 points were in care with the provider. The record sample size consisted of 3 children's records and the provider's record and the household member's record. The children in care were observed engaging with the provider and participating in nap. There have been no injuries since the last inspection and no medications are being administered at this time. Areas of noncompliance are noted in the violation narrative and were discussed with the provider during the exit meeting at the conclusion of the inspection.

Standard #: 8VAC20-800-60-B
Description: Based on record review and determined the provider did not ensure that each child's record contained all the required information.
Evidence:
1. The record for child #1 did not contain the required emergency contact information.
2. The record for child #3 did not contain documentation of annual review of the child's emergency contact information.
3. The provider confirmed that these record did not contain the required information.

Plan of Correction: Provider will request that information from the parents.

Standard #: 8VAC20-800-180-A
Description: Based on record review and interview it was determined the provider did not ensure that each household member had an updated TB screening form.
Evidence:
1. The record for household member did not contain documentation of an updated TB screening. The document in the record was dated 8/23/21.
2. The provider confirmed that there was not an updated TB screening available.

Plan of Correction: Household member will complete an updated TB screening.
