Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on 8/31/2020 and concluded on 9/1/2020. The in-charge person was contacted by telephone to initiate the inspection. There were 20 children present and 5 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 3 children?s records and 3 staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.
Based on a review of children's records, the center did not maintain a record for each child enrolled which contained the following information:
Evidence: Child #3's (age: 18 months) record did not contain documentation of a work telephone number for the father, information for a second emergency contact person, and a date on the physical and immunization records.
Plan of Correction:
I will contact parents and have them resubmit the forms.
