Comments:
An unannounced Subsidy Health & Safety inspection was conducted today, July 30, 2019 to determine the center's compliance with Subsidy Health and Safety vendor requirements. The inspection was initiated at 2:10pm and concluded at approximately 5:20pm. Approximately twenty-five children were present being supervised by five staff with the correct child/staff ratios. All subsidy requirements were reviewed and compliance determined. Administrative records and all areas in the center used by children were inspected. See attached notice for violations found during this inspection.

Based on the inspection of administrative records, the required allergy plan was not available to review.
Evidence:
During the inspection of five children's records, it was discovered that child #3 with a listed allergy did not have any documentation of an allergy plan. Subsidy vendor requirements stipulates for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and steps to be taken in the event of a suspected or confirmed allergic reaction.

Based on the inspection of administrative records, the required sworn statement form was not available to review.
Evidence: During the inspection of five staff records, it was discovered staff #5 did not have a sworn statement form in the record. Subsidy vendor requirements stipulate for each staff employed to complete a sworn statement form within 30 days of the individual's beginning date of employment or service.
