Technical Assistance:
An exit interview was conducted with the director to discuss the results of today's inspection. Consultation was given regarding updated regulations in child/staff ratios, CPR certification, out of state searches, and accessing the department's website for notices, forms, and additional information. A copy of the new regulations was given to the director during this inspection.
Comments:
An unannounced monitoring inspection was conducted today, July 30, 2019 to determine the center's compliance with religious exempt requirements. The inspection was initiated at 2:10pm and concluded at approximately 5:20pm. Today approximately twenty-five children were in attendance being supervised by five staff with the correct child/staff ratio in each classroom. Administrative records, children's classrooms, restrooms, common areas, and outdoor playground were inspected. See attached notice for violations found during this inspection.

Based on the inspection of administrative records, the required sworn statement form was not available to review.
Evidence: During the inspection of five staff records, it was discovered staff #5 did not have a sworn statement form in the record. Religious Exempt requirements stipulate all applicants for employment, employees, applicants to serve as volunteers, and volunteers and any other person who is expected to be alone with one or more children enrolled in the child day center are to complete the required sworn statement form.

Description:
Based on the inspection of administrative records, the required annual staff health report was not obtained.
Evidence:
During the inspection of five staff records, it was discovered that staff #3 did not have an updated annual staff health report in the record. The last staff health report on file dated 07/23/18 expired on 07/23/2019.

Description:
Based on the inspection of the building the center did not ensure that children are free of obvious hazards.
Evidence:
During the inspection of the building, chipped and peeling paint was observed above the three sinks and cabinet located in the hallway. Chipped paint/peeling was also observed on the opposite wall from sinks and the lower wall in the cafeteria.
Plan of Correction:
Per administrator: We will have these areas repainted soon as possible.
