Comments:
The Licensing Inspector conducted an unannounced Subsidy Health and Safety inspection on Wednesday July 21, 2022 from approximately 10:30 AM to 12:45 pm. There were 17 children in attendance and a total of four staff supervising. A total of 6 children?s files and 6 staff files were reviewed. A code compliance inspection was conducted on the same date as the SHSI supplemental inspection.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-790-640-A
Description: Based on observations, the vendor did not ensure all hazardous substances such as cleaning materials were kept in a locked place using a safe locking method that prevented access by children. Evidence: A container of disinfecting spray was observed sitting on a shelf in the enclosed Infant outdoor play area.

Plan of Correction: Corrected during the inspection.

Standard #: 8VAC20-790-660
Description: Based on observation, the vendor did not ensure that all areas of the premises accessible to children are free of obvious injury hazards, including providing and maintaining sand or other cushioning material under playground equipment.
Evidence: Two slides located on the playground was sitting on the bare ground.

Plan of Correction: I will discuss with the pastor about what type of cushioning material she wants to add underneath the slides and we will add it.

Standard #: 8VAC20-790-770-D
Description: Based on observation, the vendor did not ensure medication, except for those prescriptions designated otherwise by written physician's order, including refrigerated medication and staff's personal medication were kept in a locked place using a safe locking method that prevented access by children.
Evidence: A first aid kit was sitting on top of a cabinet in the Two's classroom. Various individual packets of over the counter medications, including aspirin and antacaids were located in the first aid kit. The first aid kit was not kept in a locked location.

Plan of Correction: Corrected during inspection. Moved to a locked location while the inspector was on site.
