Comments:
An unannounced subsidy health and safety inspection was conducted Wednesday, August 28, 2019 from approximately 1:30 PM until 4:00 PM to determine the center's compliance with subsidy vendor requirements. During the inspection there were 34 children present, with the following staff-to-child ratios being followed:
Infants: 6 children with 1 staff (see violation notice)
Twos: 6 children with 1 staff
Threes & Fours: 7 children with 1 staff
School Age: 15 children with 1 staff

Five children?s records and five staff records were reviewed. A licensing inspection was also conducted this date. Violations were found during the subsidy health and safety inspection and are identified on the violation notice. If you have any questions, please contact the licensing inspector at (804) 662-9790.

Violations:
Standard #: 22VAC40-665-520-B
Description: Based on the review of five children's records, the vendor did not ensure the records for three children contained the name, address, and telephone number of at least one person designated by the parent to contact in case of an emergency and the parent cannot be reached. Evidence: The records for Child #2, Child #3 and Child #4 did not contain the address for the emergency contact listed in the record.

Plan of Correction: We will ask for the addresses for the emergency contacts when the families pick the children up this afternoon.

Standard #: 22VAC40-665-530-2-d
Description: Based on review of five staff records, the vendor did not have documentation for one staff member who began employment or service after the vendor agreement had been signed in the file within 30 days of the staff?s beginning date of employment or service.

Evidence:
The record for Staff #2 contained satisfactory results of a national fingerprint based criminal record check dated 06/05/18. The record indicated Staff #2 original hire date was 05/19/14 and separated from employment 08/16/18. Staff #2 was then rehired on 03/11/19. The original results of the national fingerprint based criminal record check were not re-done.

Plan of Correction: We will have Staff #2 complete the national background check again.

Standard #: 22VAC40-665-540-A
Description: Based on a review of five staff records, two staff members did not submit documentation that they were free of communicable tuberculosis (TB) when evaluated by a health professional within thirty days prior to employment and prior to coming in contact with children. Evidence: The record for Staff #1 (hired date 01/02/19) contained a negative TB screening dated 01/10/19. The record for Staff #2 (hire date 03/11/19) contained a negative TB screening dated 04/08/19.

Plan of Correction: Going forward, we will ensure staff have TB screenings prior to beginning employment and within the required time frame.

Standard #: 22VAC40-665-650-E
Description: Based on interview, the vendor did not develop and implement a written policy and procedure that describes how the vendor will ensure that each group of children receives care by consistent staff or team of staff members. Evidence: A member of management stated there has not been a policy developed for care by consistent staff or team of staff members.

Plan of Correction: We will develop a written policy to meet this requirement.

Standard #: 63.2(17)-1716-A-3
Description: Based on observation and inspection of the facility, the vendor did not ensure that ratios were maintained for children from birth to the age of 16 months: one staff member for every four children.

Evidence:
In the Infant classroom, there were six children (six months to 18 months of age) being cared for by one staff member. A volunteer was present but did not meet the requirements to be a staff member.

Plan of Correction: A member of management will go into the classroom while we are in the process of hiring additional staff members.
