Comments:
An unannounced monitoring inspection was conducted from 4:00 pm - 5:00 pm. There were 35 children in are being supervised by 3 staff. The children were observed participating in table-top activities. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and six staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on review of medication it was determined that not all procedures regarding prescription medication were followed. Evidence: The emergency medication on site for child A and child B did not have a written authorization from a parent or guardian.

Administration will request written authorization for medication forms from the parents of child A and child B.

Description:
Based on review of medication it was determined that medication was not picked up by a parent within 14 days after a written authorization for medication expires. Evidence: The medication on site for child C was accompanied by a written authorization for medication form which expired on 09/28/23.

Administration will request a new written authorization for medication form from the parents of child C or the medication will be sent home with the parents.

Description:
Based on observation and interviews it was determined that not all required components of the first aid kit were operable. Evidence: There were only three of the six required batteries on site to operate the required radio for the first aid kit.
Plan of Correction:
Administration will purchase batteries for the radio.
