Comments:
An unannounced monitoring inspection was conducted from 7:20 ? 8:10 am. There were 15 children, ranging in age from five years to ten years of age, present with two staff supervising. The children were observed having free play in the gym. Five children's files were reviewed. Four staff records were reviewed offsite at the Massad central office on 9/26/19 and were in compliance. The emergency drill log and first aid kit were observed and in compliance. The last health inspection was conducted: 3/8/19 and the last fire inspection was conducted: 8/26/19. Based on a review of five children's files, the center did not obtain documentation that each child has received immunizations prior to the child's first attendance. Evidence: Child #5 (date of first attendance: 09/04/2018) did not have documentation of immunizations in their file.
