Comments:
An unannounced monitoring inspection was conducted from 5:15 - 6:15pm with the Assistant Manager. There were nine children, ranging from five years to ten years of age, supervised by three staff. Children were observed cleaning up after snack and having free play in the gym. Four children?s files were reviewed and in compliance. Three on-site staff files were reviewed. Three medications and authorization forms were reviewed and in compliance. The last fire drill was conducted: 10/21/19 and the last shelter-in-place drill was conducted: 8/20/19. Based on review of documentation, and an interview with staff, there was not a staff member present with current Medication Administration Training (MAT) certification. Evidence: staff #1's MAT certification was dated 5/16 and it expired in 5/19. Three medications were on site.
Plan of Correction:
Staff stated that she completed the MAT renewal class but has not received her certificate yet. Another staff member arrived at 5:45 pm with current MAT certification.

Description:
Based on review of documentation and staff interview, there was not at least one staff member trained in first aid and cardiopulmonary resuscitation and rescue breathing present (CPR). Evidence: there was no documentation that any of the three staff present had current certification in first aid/CPR.
Plan of Correction:
Staff have CPR/First aid training - will locate documentation.
