Technical Assistance:
Discussed different emergency situations, lock down drills vs shelter-in-place drills, and consulting with law enforcement to strategize best practices for conducting lock down drills at this site.
Comments:
An unannounced monitoring inspection was conducted from 5:10 - 5:40 pm with staff. There were six children, ranging from five years to 11 years of age, present with three staff supervising. Children were observed playing in a variety of centers with age-appropriate toys. Three onsite staff records and two onsite child records were reviewed at the time of the inspection along with the daily attendance record, emergency drill log, written list of food allergies, sensitivities and dietary restrictions, two medications and the required posted information. Staff on site have current Medication Administration Training (MAT) certification. Three staff records were reviewed off site at the YMCA office on 5/4/2023. Based on review of three staff records, it was determined that not all staff had an Office of Background Investigations (OBI) fingerprinting-based criminal history record check every five years as required. Evidence: The record for staff #1 had a OBI determination letter dated: 3/23/18.
Plan of Correction:
Staff was sent to obtain an updated fingerprint-based criminal history report.
