Comments:
A monitoring inspection was conducted on October 24, 2022. There were no children present. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, and medication. A total of four children?s records and one caregiver record were reviewed. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22.1-289.036-A
Description: Based on review, provider did not ensure there is required background checks completed every 5 years.

Evidence: Provider's central registry search was completed 8/9/2016 which is more than 5 years old.

Plan of Correction: Provider has submitted a central registry search twice. Provider verified with the central registry unit that the most recent request was received and is pending results.

Standard #: 8VAC20-850-110-C
Description: Based on observation, interview and review, provider did not ensure the criteria is met on the health and safety checklist.

Evidence: The following was found during inspection:
1. Multiple bottles of toxic household products including disinfectant sprays, bug spray and carpet cleaner were stored on the floor in the living room. The items were all accessible to children.
2. There was not soap available in the hall bathroom used by children. Provider stated children use hand sanitizer instead of soap.
3. The record for child #3, start date 12/28/2021, did not have documentation of viewing proof of child's identity and age.

Plan of Correction: 1. Chemicals were all moved and will be kept in the pantry or on top of the refrigerator where the children cannot reach them.
2. Soap will be placed and kept available in the bathroom.
3. Birth certificate will be viewed and information documented in child's record.
