Comments:
A monitoring inspection was conducted on September 5, 2023 from approximately 12:05 pm to 1:15 pm to review voluntary registration requirements. There were no children present. The inspector reviewed compliance in the areas of administration, physical environment and programming.. A total of two children?s records and one caregiver record were reviewed. Violations were found and are noted on the Violation Notice.
Based on interview, provider did not ensure there is an updated TB test or screening every two years.
Evidence: Provider stated she does not have a current TB test or screening completed within the past two years but she knows one needs to be done. Provider could not find documentation of most recent TB test or screening for review.
Plan of Correction:
Provider will schedule appointment today to have a TB test or screening completed.

Repeat Violation Based on review, provider did not ensure 1 of 2 child records has documentation of required information.
Evidence: The record for child #1 (start date 7/3/2023) did not have father's home phone number, mother's employment phone number and a physical examination.
Plan of Correction:
Parents' home and employment information was added on 9/6/2023. Parents will be informed a physical or well baby check documentation is required.
