Comments:
An announced monitoring inspection was conducted on October 31, 2019 to review voluntary registration requirements. No children were present. Two children are currently enrolled. Provider record and two children records were reviewed.
Based on review, provider did not ensure 1 of 2 children's records has documentation of required information.
Evidence: The record for child #2 did not have father's home address.
Plan of Correction:
Provider will call the parent and ask her to bring the information today.
