Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program. A monitoring inspection was initiated on 09/13/21021 and concluded on 09/16/2021. The director was contacted by telephone to initiate the inspection. There were 59 child present and 16 staff. The inspector emailed the director/provider a list of items required to complete the inspection. The Inspector reviewed 4 children?s records and 4 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.
Based on observation the center did not ensure areas and equipment of the center, inside and outside were maintained in a safe condition. Evidence: The metal stair case the school-age children use to exit to the playground and for emergencies had rust on one of the steps.
Plan of Correction:
The following corrective action was provided by the director: will contact DM and get maintenance to remove rust.

Description:
Based on observation the center did not ensure all requirements were met when diapering. Evidence: A staff member in the infant room was observed changing a child's diaper. The staff member did not dispose of the soiled diaper in the foot operated trash can. The soiled diaper was wrapped up and placed on the changing table while putting a clean diaper on the child.
Plan of Correction:
The following corrective action was provided by the director: staff that are responsible for diapering were immediately notified and retrained.
