Comments:
An unannounced monitoring inspection was conducted on 5/22/24 from 4:10 p.m. to 5:00 p.m. with the Site Director. There were 26 children in care, ranging in age from five to 12-years-old, supervised by three staff. The children were observed playing with table toys. Five child records and three staff records were reviewed. The Center has three staff with current certification in CPR and First Aid, and three staff trained in Daily Health Observation Training. No medications were on site and the center has two staff current in Medication Administration Training (MAT). Required postings were observed. The attendance, menu, and emergency drill log were reviewed. The first aid kit, flashlight, and battery-operated radio were observed. Areas of non-compliance are identified in the Violation Notice. Based on review of five staff records, the center did not obtain documentation of the results of a central registry search within 30 days of employment for all staff members.
Evidence: 1. The record for Staff #1 with an employment date of 8/24/22, the results of the central registry search background check was dated 10/24/22. 2. The record for Staff #2 with an employment date of 6/29/22, the results of the central registry search background check was dated 9/22/22. 3. The record for Staff #3, with an employment date of 9/13/22, the results of the central registry search background check was dated 10/26/22.
Plan of Correction:
When waiting for central registry search background, there will be an email sent out to check on when results will be received.

Description:
Repeat Violation
Based on review of the written attendance record, the center did not ensure they maintained a written record of daily attendance for each group of children documenting arrival and departure of each child in care as it occurs.
Evidence: At approximately 4:10 p.m. there were 26 children present. Staff stated there were 30 children in attendance and the written record reflected 28 children present.
Plan of Correction:
Staff meeting was held to address the issue and to express the importance of knowing our head count vs attendance.
