Comments:
An unannounced monitoring inspection was conducted on January 3 2020 between 10:30am and 11:45am. There were 50 children in the direct care of 8 staff members. Children were observed eating lunch, washing hands, playing on electronics, and playing games. The center is clean and organized. Areas of non-compliance are listed on the violation notice. Based on record review, 1 of 3 staffing records reviewed did not contain documentation that an out of state central registry search was requested for any staff member that resided outside of the Commonwealth of Virginia within the last 5 years.
Evidence: Staff #1 has listed on his sworn statement that he has lived in another state within the last 5 years. Documentation of a request was not found in the record.
