Comments:
An unannounced monitoring inspection was conducted between the hours of 9:00am and 12:00pm. Upon arrival there were 66 children in care with 16 staff present. A sample of 2 staff and 6 children's records were reviewed in addition to 4 medications. Children were observed in the gym, playing in learning centers and being read a story. Infants were observed on the floor with staff and one was observed sleeping in a crib.
Based on a record review the center did not ensure all required information was kept for each enrolled child. Evidence: Child #4 (enrolled 09/03/19), did not have documentation of 1 of the 2 required emergency contacts.
Plan of Correction:
The following corrective action was provided by the director during the exit interview: A second emergency contact will be obtained and documented today at pick-up.

Description:
Based on observation the center did not have all of the required emergency supplies. Evidence: In the emergency preparedness bag, the center's extra batteries for the emergency radio were corroded and not able to be used.
Plan of Correction:
The following corrective action was provided by the director during the exit interview: This was an oversight. New batteries will be placed in the emergency kit today.
