Comments:
An unannounced monitoring inspection was conducted on September 18, 2019. Upon arrival to the center there were 15 children present with two staff members in the cafeteria having lunch. Today's lunch consisted of hot dogs on buns, baked beans, spinach, mixed fruit and milk. The children were also observed during toileting, handwashing, and playing in centers. The licensing inspector reviewed the physical plant, administration, required postings and documentation, health and safety supplies and procedures, nutrition and supervision. Two staff records were reviewed. Five children's records were reviewed. One medication was reviewed. Exit interview was conducted with the Program director/Lead Teacher.
Based on record review, in two of five children's records reviewed, the center did not maintain a record for each enrolled child which contained the following information:
Evidence: The Lead Teacher confirmed that there were not complete addresses for two emergency contacts in the record for Child #1 and there was not a second emergency contact name, address and phone number in the record for Child #4.
Plan of Correction:
We will ask the parents to provide the information.
