Comments:
An unannounced monitoring inspection was completed on 10/09/2019. There were 39 children, ages 4 years-10 years, and 5 staff present during the inspection. The inspector reviewed 5 children's records, 4 staff records and 4 medications during the inspection. Children were observed in the following activities: during organized large motor play in the gym, during homework time, having an afternoon snack and participating in arts and crafts. The inspector reviewed the emergency supplies during the inspection. The inspector discussed subsidy requirements and table sanitizing products with the staff. Based on record review and discussion with the staff, the facility failed to ensure that staff records shall contain all of the elements as required by the standards.
Evidence:Staff #3 confirmed that there was no record for Staff #5 on-site, confirming that there was no documentation of the name, address and telephone number of a person to be notified in an emergency which shall be kept at the center for Staff #5..

Description:
Based on observations and discussion with the staff, the facility failed to ensure that when children are regularly in ongoing mixed age groups, the staff-to-child ratio applicable to the youngest child in the group shall apply to the entire group.
Evidence: There were 23 children, ages 4 years-9 years, with 2 staff members in Group 1. Staff #2 confirmed that Group 1 has 4 year old-9 year old children in the group every day. The applicable ratio for this regularly ongoing mixed age group is one staff member for every 10 children.
Plan of Correction:
Children will be moved to Group 2 to meet ratio.

Description:
Based on document review and discussion with the staff, the facility failed to implement a monthly practice evacuation drill.
Evidence: There was not a monthly practice evacuation drill conducted in September 2019. Staff #3 verified that a drill was not conducted in September 2019.

A drill will be conducted monthly.
