Technical Assistance:
Inspector spoke with provider about submitting a renewal application for the family day home during this inspection.
Comments:
An unannounced monitoring inspection was conducted today from 11:50 am to 12:30 pm. At the time of inspectors' arrival there was one child aged 18 months(3 points) in care with the provider. The child in care was observed interacting with the provider throughout this inspection. A sample of 1 child's records and 2 provider/household member records were reviewed. There were no injuries since the last inspection. Provider reported that no medications are administered at this time. Areas of noncompliance are identified on the violation notice and were discussed with the provider during the exit interview at the conclusion of the inspection.
Based on record review it was determined the provider did not obtain documentation of additional immunizations every six months for children under the age of two years. Evidence: 1. The record for child #1 did not contain documentation of additional immunizations every six months. The documentation available was date 5/31/19. 2. The provider confirmed that updated immunization documentation was not available.

Provider will request updated immunization documentation from the parent at pickup.
