Comments:
A monitoring inspection was conducted today, between 9:00am and 10:15am. There were children present, with 2 caregivers supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of4 child records and 2 caregiver records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on review of children's records, the family day home did not obtain documentation of additional immunizations every six months for a child under the age of two. Evidence Child #1, first day 8/17/22, and #2 first day 5/27/22, are both under the age of two, do not have documentation of additional immunizations every six months. Child #1 last immunization documentation dated 8/23/22. Child #2 last immunization documentation dated 9/15/22.
Plan of Correction:
I will get an updated immunization record and make sure to get one every six months for all children under the age of two.
