Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on September 16, 2020 and concluded on September 18, 2020 The director was contacted by telephone to initiate the inspection. There were 71 children present and 14 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 5 children?s records and 5 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.
Based on review of five children's records, one is incomplete. Evidence: Child #4 does not have documentation of immunizations before first day of attendance.
Plan of Correction:
We will get a copy of the immunization record and in the future before a child starts we will get the record.

Description:
Based on review of five children's records, two are incomplete. Evidence: Children # 2 and #3 do not have documentation of parental annual update of child's information. Last update for both children was 2018
Plan of Correction:
We will develop a plan to obtain parental annual updates.
