Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.
A monitoring inspection was initiated and concluded on October 14, 2021. The provider was contacted by telephone and a virtual inspection was conducted. There were six children(18pts) present, ranging in ages from 10 months to 2 years old, with two staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of two child records and two staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review, children's records did not contain the required information. Evidence: 1. Child #1's record(Start date-08/02/2021) did not contain documentation of the Provisions of the Emergency Plan.
Plan of Correction:
The provider will give the parents a copy of the Provisions of the Emergency Plan and have the parents to sign it.

Description:
Based on record review and interview, the caregiver did not obtain current certification in cardiopulmonary resuscitation (CPR), as appropriate to the age of the children in care. Evidence: 1. Staff #1 did not have documentation of current certification in CPR. The CPR expired on 03/16/2021. 3. Staff #1 stated that the certification had expired and is actively looking for classes.
Plan of Correction:
The provider has been looking for CPR classes to take. The provider will take a CPR class and renew the certification

Description:
Based on record review and interview, the provider did not obtain for each caregiver a current Report of Tuberculosis Screening form, in accordance with the requirements in 22 VAC 40-111-170, every two years from the date of the first screening. Evidence: 1. Staff #1 did not have documentation of a subsequent tuberculosis screening in the record. The last tuberculosis test was dated 03/14/2019. 2. Staff #1 stated that a current tuberculosis screening had not been completed.
Plan of Correction:
The staff member will get a tuberculosis screening from the doctor.
