Comments:
An unannounced monitoring inspection was conducted from 12:00 noon ? 1:30 pm with the provider and her assistant. There were five children in care, ranging from two years to four years of age, for a total of ten points. The children were observed eating lunch and reading a wide variety of books before naptime. One child was reading his sight words. Lunch served today was ham, creamed corn, apple sauce, whole wheat bread and milk. Four children's records and three provider/household member records were reviewed and in compliance. Required information was posted in addition to variety of seasonal artwork. The last emergency escape drill was conducted: 11/27/19, the last shelter-in-place drill was conducted: 9/3/19 and the last lockdown drill was conducted: 5/6/19. The provider is not Medication Administration Training (MAT) certified and cannot administer medication to children in care. Standard #: 22VAC40-111-180-A
Description: Based on a review of three provider/household member records, and an interview with the provider, not all caregivers had a repeat tuberculosis (TB) test/screening at least every two years from the date of the previous screening. Evidence: The last TB test/screening for provider #2 was dated: 8/30/17.

Plan of Correction: Will send the provider for an updated TB test/screening no later than December 20, 2019.
