Comments:
An unannounced monitoring was conducted on 4/25/24 from 8:15 a.m. to 9:10 a.m. with the provider. There were 11 children in care, ranging in age from two to five-years-old, for a total of 20 points. Children were observed playing with manipulatives. Five child records, two staff records, and one household member records were reviewed. The provider has current certification in CPR and First Aid. No medications were on site and the provider understands that they must obtain certification in Medication Administration Training (MAT) in order to give medication. The first aid kit, flashlight, weather band radio, and fire extinguisher were observed. The emergency drill log, attendance, and menu were also reviewed. Areas of non-compliance are identified in the Violation Notice. Based on a review of records and interview with the provider, the provider did not obtain subsequent tuberculosis (TB) screening, every two years from the date of the initial screening.
Evidence: The last documented TB test for Staff #1 is dated 4/8/22, Staff #2 is dated 04/11/22 and household member #1 is dated 4/8/22.
