Comments:
A Subsidy Health and Safety Inspection (SHSI) was conducted on 6/28/2024 from 1:45pm to 3:13pm. A licensing inspection was conducted on the same date as the SHSI inspection. The two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements not covered in licensing standards were reviewed during this supplemental inspection. The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice and discussed during the exit interview.

Standard #: 8VAC20-790-600-D-1
Description: Based on record review and interviews, it was determined the vendor did not ensure that all staff who work directly with children have within 90 days of the date of employment or 90 days from subsidy vendor approval, current certification in cardiopulmonary resuscitation (CPR) appropriate to the ages of children in care.
Evidence:
The CPR certification in the record for staff #2 expired 9/2023.

Description: Based on record review and interviews, it was determined the vendor did not ensure that all staff who work directly with children shall have within 90 days of the date of employment or 90 days from subsidy vendor approval current certification in first aid appropriate to the ages of children in care.

Description: Based on record review and interviews, it was determined the vendor did not ensure staff who work directly with children shall, in addition to preservice and orientation training required, staff will annually attend at least 16 hours of training and staff development activities, to include the departments health and safety update course.
Evidence:
1.Staff #1, staff #3 and staff #5 did not have documentation in the record that the required annual health and safety update training had been completed for 2023.
2.The Director confirmed the documentation was not in the record.
