Comments:
his inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews, and video tour.
A monitoring inspection was initiated on 8/24/2021 and concluded on 8/27/2021. The provider was contacted by email to initiate the inspection. There were 10 children present with the provider and assistant provider. The inspector emailed the director/provider a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 2 provider/household member records, along with any requested program records submitted by the facility to determine if required documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.
Based on record review and interview it was determined the provider did not obtain a report of tuberculosis screening form from each caregiver every two years from the date of the first screening. Evidence: 1. The last TB screening report for the assistant provider was dated 3/7/19. 2. The provider confirmed that the assistant did not have an updated TB screening.
Plan of Correction:
Assistant provider will get an updated TB screening report.
