Comments:
An unannounced on-site monitoring inspection was conducted on 7/7/2022. There were 143 children, ages3 months--12 years, and 22 staff members and the director present during the inspection. The inspector observed the following: morning arrival, snack, free choice activities, diaper changing, and water play. A total of 5 children?s records and 5 staff records, were reviewed. The inspector arrived at 9 AM and departed at 11:15 AM.

Based on review of five children?s records the center failed to ensure that all records were complete per the standard.
Evidence: 1. Child # 1 did not have, chronic physical problems completed on the enrollment form by the parent or the guardian. 2. Child # 3 was missing father?s information, chronic physical problems and any noted allergies.

The center director will obtain the missing information ASAP.
