Comments:
An unannounced monitoring inspection was conducted today from 9:15am-10:30am. There was 1 child directly supervised by 2 caregivers. The home, 2 caregiver record, 5 children?s records, 1 household member?s records, evacuation drills, emergency supplies, and policies were inspected. The child was observed playing with a caregiver. There was an assortment of age appropriate materials for children. Areas of non-compliance are identified in the Violation Notice. Based on review of children's records, the providers did not obtain documentation of a physical examination under the direction of a physician within 30 days after the first day of attendance. Evidence The record for child #1 (first day of attendance 5/1/23) and child #2 (first day of attendance 8/15/22) do not contain documentation of a physical examination under the direction of a physician within 30 days of first day of attendance.

I will get the physicals and make sure all the physicals are signed by a doctor before accepting them.

Description:
Based on observation, the diapering surface is not nonabsorbent. Evidence The diapering surface is ripped exposing nonabsorbent materials.
Plan of Correction:
I ordered a new diapering pad and will use the other diapering surface until the new one arrives.
