Comments:
An unannounced monitoring inspection was conducted beginning at 11:45 a.m. and ending at 1:30 p.m. with the provider and assistant. There were 11 children in care ranging in age from 6 months through 4 years of age for a total of 24 points. Children were observed watching an age appropriate television program in the livng room area, playing on a cellphone and building blocks. Chidlren than washed hands and prepared for lunch. Lunch today was spaghetti and sauce, chicken nuggets, clementines and apple slices and milk. After lunch, children prepared for nap.
Four children's records and two caregiver records were reviewed. Emergnecy drills were reviewed and up to date. The weekly menu was posted.

Based on observation, areas and equipment of the family day home were not maintained in safe, operable condition. Evidence: In the front area of the childcare, there are several areas of flooring missing underneath the area rug that create a tripping hazard. A play kitchen had an exposed screw that extended approximately 10 thread lengths that created an impalement and scratch hazard. A carpet cleaner was observed with an unsecured, loose cord that created an entanglement and tripping hazard.

Description:
Based on observation, two cribs did not have tight-fitting bottom covers. Evidence: One crib was observed with a loose fitting sheet on top of two thin mattresses that were placed on top of eachother. A second crib had a loose fitting sheet over a matress that was topped with a thick comforter type blanket.
