This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contact, documentation review, interview, and a virtual inspection.
An inspection was conducted on December 29, 2021 and January 7, 2022 to investigate a self-reported incident regarding compliance with Staffing and Supervision and Programs. The provider was contacted by telephone and video call. A virtual inspection was conducted on January 7, 2022.
The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
