22VAC40-180 HEALTH AND SAFETY CRITERIA AND CHECKLIST.

Comments:
A monitoring inspection was initiated on 3/17/2022 and concluded on 3/18/2022. There were two children present and two staff. The Inspector reviewed two children?s records and the provider and household member?s records for updates. The inspector reviewed and observed for compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 8VAC20-850-90-C
Description: Based on record review and interview the provider failed to maintain an individual record for each child enrolled that contained all the required information.

Evidence:

1. The record for child 1 (start date 10/1/21) did not contain the following: The home address for the father; The name, address and phone number of the place of employment and work hours for the father; The hospitalization plan number for the child; The parent's signed authorization to use a substitute provider and his or her name, address and phone number; and The disclosure of whether the family day home has liability insurance coverage.
2. The record for child 2 (start date 2/14/22) did not contain the following: The results of health examination and up-to-date immunization records; The parent's signed authorization to use a substitute provider and his or her name, address and phone number; and The disclosure of whether the family day home has liability insurance coverage.
3. The provider verified the items were not obtained for children 1 and 2.

Plan of Correction: All information missing for child 1 and child 2 will be obtained. In the future the provider will ensure all information is obtained prior to the first date of attendance.

Standard #: 8VAC20-850-110-C
Description: Based on record review and interview, the provider failed to practice monthly evacuation drills.

Evidence:

1. The documentation of monthly evacuation drills did not have documentation of drills for the months of 7/21, 11/21 and 12/21.
2. The provider verified drills were not completed those months as the home was not open for full months and they were unable to fit in drills.

Plan of Correction: In the future emergency drills will be completed every month. If the home will be closed part of the month the drills will be fit in.
