Comments:
An unannounced monitoring inspection was conducted on October 28, 2019 from approximately 11:10 am to 12:40 pm. Eight staff were present with thirty-seven children. Additional staff were on site for kitchen and administrative duties. Five children and five staff records were reviewed.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on review, facility did not ensure 1 of 5 children's records has documentation of required information.

Evidence: The record for child #3 (start date October 14, 2019) did not have mother's employment information and two individuals to contact in the case of an emergency when parents cannot be reached. Information was verified by program director.

Plan of Correction: This particular child is no longer in our center however when she was the parents expressed they have no family here being in a refugee situation and the child could not speak any english. Mother expressed that she or or her husband would always be available.

Standard #: 22VAC40-185-500-B
Description: Based on observation, facility did not ensure disposable diapers are disposed in a storage system that is either foot operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal.

Evidence: Soiled disposable diapers were observed in a plastic bag hanging from a door knob by the diapering table in the toddler classroom. Staff stated she had recently changed the children's diapers.

Plan of Correction: All staff has been retrained in regards to properly disposing of soiled diapers.
