Based on review of five staff files and staff interview, the facility failed to have 2 out of 5 staff complete the Virginia Department of Education-sponsored orientation course (VA Pre-service) within 90 days of employment. Evidence: Staff 3 (date of employment 11/30/2021) and Staff 4 (date of employment 6/09/2022) did not have documentation of having completed the VA Dept of Education sponsored orientation course within 90 days of employment. Management stated that the staff did not have documentation of the training.

All staff will complete the 10 hour pre-service training within 30 days of employment.

Evidence: Staff 1, Staff 2, Staff 3, Staff 4, and Staff 5 were on site with 27 children on 5/08/2023. None of the 5 staff have documentation of current CPR on file. Management stated that 5 out of 5 staff do not have current CPR certification.
