An unannounced monitoring inspection was conducted Tuesday, September 17, 2019 to determine the center's compliance with religious exempt requirements. The inspection was initiated at approximately 10:00 AM and concluded at approximately 1:30 PM. A total of seven children were present during the inspection and being supervised by three staff members. All required staff-to-child ratios were being maintained. Children were observed learning about their shapes and colors, singing, playing a memory game, watching children's programs on T.V., washing hands and taking bathroom breaks, eating lunch and resting at nap time. All areas of the center were inspected including the classrooms, hallways, bathrooms and outdoor play area. Five children?s records and four staff records were reviewed. Policies and procedures were discussed and information disclosed to parents, guardians and public was reviewed. Violations were cited during the inspection. Based on review of four staff records and interview, the center did not ensure a person trained and certified in first aid and cardiopulmonary resuscitation (CPR) was present at the child day center whenever children were present or at any other location in which children attending the child day center were present. Evidence: The records for Staff #1, Staff #2, Staff #3 and Staff #4 contained CPR cards that expired on 08/29/19. Staff #1 stated they have not had a CPR training since the certification had expired. Currently the center has four staff employed.
Plan of Correction:
We will schedule a training for CPR and try to find someone who offers CPR training that is valid for two years instead of one year.

Description:
Based on the review of five children's records, the center did not establish and implement procedures to ensure that one child in the center is in compliance with the provisions of ? 32.1-46 regarding the immunization of children against certain diseases. Evidence: The record for Child #2 (first day of attendance 08/12/19) did not have documentation of immunizations. Staff #1 confirmed this information while the inspector was on site. The parent handbook states ?immunization records must be signed by each child?s physician or Health Department prior to his/her first day of attendance?.
Plan of Correction:
We have requested the immunization record from the parent. We will ask the parent again for the record.

Description:
Based on review of four staff records and interview, the center did not obtain satisfactory results of the finger-print based national criminal background check for one staff prior to beginning employment. Evidence: The record for Staff #4 (hire date 08/18/2018) contained satisfactory results of the finger-print based national criminal background check dated 08/30/2018.
Plan of Correction:
We will make sure in the future staff do not start before we receive the results of the national background check.
