The licensing inspector conducted an unannounced subsidy complaint inspection in response to a complaint that was received by OCCHS on April 20, 2023. The inspection was initiated on May 4, 2023 and completed on May 4, 2023. On May 4, 2023, the inspector was on site from approximately 8:48am to 10:00am. Interviews with staff were completed, observations were made at the center, and documentation was reviewed. The preponderance of the evidence gathered during the investigation does not support the allegation; therefore, the complaint is determined to be not valid.
