Based on record review, it was determined the provider did not obtain documentation of immunization before a child attends the family day home. Evidence: Child #4 started on 5/22/23and the immunization record is dated 6/19/23.
Plan of Correction:
I will be sure to have the immunization record on the first day with all new enrollments.
Standard #:
8VAC20-800-690-D
Description:
Based on observation and interview, the diapering surface was not sanitized after each use.
Evidence: 1. There was no sanitizing spray bottle available to disinfect the diapering surface after diapering a child. There was only a container of disinfectant wipes available for use. . 2. Staff #3 used disinfectant wipes to sanitize the diapering surface after changing child #5 diaper. 3. The provider acknowledged they were using the disinfecting wipes after cleaning the diapering surface with soap and water.
Plan of Correction:
We will start using a spray disinfectant instead of the wipes.
