Comments:
An unannounced monitoring inspection was conducted today between the hours of 2:50pm and 3:48 pm. There were 7 children (17 points) directly supervised by the provider and one assistant caregiver. The home, 2 caregiver records, 5 children?s records, 1 household member?s record, evacuation drills, and emergency supplies were inspected. Children were observed waking up from nap and having snack. Handwashing procedures were also observed. Areas of non-compliance are identified in the Violation Notice. Description:
Based on record review, the provider did not obtain documentation that one child has been adequately immunized before the child's first date of attendance.
Evidence: 1. Child #2 began attending the family day home on 11/21/2022. 2. The immunizations in the record for Child #2 were dated 12/13/2022.

Immunizations will be obtained before a child's first day.
