Comments:
A Subsidy Health and Safety inspection was conducted on 01/16/2020 to review supplemental health and safety requirements. An unannounced monitoring inspection was conducted today from approximately 3:40pm-5pm. There were 12 children (35 points) in the direct care and supervision of 2 caregivers. A sample of 6 children?s records, 2 caregiver's records and 5 household member's records were reviewed. The children were observed reading books, playing with toys, eating snack and being picked up by their parents. Zero new caregivers have been hired since the last inspection. Areas of non-compliance are identified in this report. The exit interview was conducted with the provider. If you have any questions, contact Kimberly.Sawyer@dss.virginia.gov. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.
Based on record review, the vendor failed to maintain a written copy of daily attendance that documents the departure of each child.
Evidence: The departure of 5 children in care was not documented on the 1/15/2019 attendance.
