An unannounced "Code of Virginia Section 63.2-1716" compliance monitoring inspection was conducted on March 12, 2020 beginning at 9:30 a.m. to 1:50 p.m. with the director. There were thirty children, ranging in age from four months to eight years old, in the care and supervision of five staff. Children were observed during nap time. Center documents and five children's records were reviewed and in compliance. Seven staff records were reviewed. There were two medications on site. Two staff were Medication Administration Training (MAT) certified. The emergency escape and shelter in place drill logs were reviewed and in compliance. The most recent lock down 3/19/19.

Based on review of documentation, the facility failed to obtain sworn statement of affirmation by the first day of hire and Central Registry Background checks within thirty days of hire. Evidence: Staff A, date of hire 11/12/2019, did not have documentation of Central Registry Background check results.
Plan of Correction:
The central registry request was filled out and in the staff record; but it was not mailed off. The central registry request will be mailed off immediately.
