Technical Assistance:
Reviewed modification process once Arlington County approves a capacity increase to 12 children. Provider can send an email request along with a copy of the Zoning approval.

Comments:
An unannounced monitoring visit was conducted today. Two caregivers were present with seven children for twenty three points which is within ratios for supervision. Observed children engaged in puzzles and free play. Reviewed three children's records and two caregiver records, emergency preparation for drills, written plan update, first aid and CPR certification as well as nutrition. Violations:
Standard #: 22VAC40-111-60-B
Description: Based on record review, one of three children's records was incomplete.
Evidence:
Child #1 was missing a second emergency contact address.

Plan of Correction: Will contact parent to complete information.

Standard #: 22VAC40-111-90-C
Description: Based on record review, 1 of 3 children's record does not contain an updated immunization record for a child under two years of age.
Evidence:
Child #2's immunization record is older than six months and the child is under two years of age. Last documented immunization was 12/2018.

Plan of Correction: The provider will contact the parent for an updated immunization record.

Standard #: 22VAC40-111-240-A
Description: Based on observation, an area of the home is not in a safe condition as protruding nails are visible.
Evidence:
The lattice is loose from the back deck area where the children play and several nails are not flush to the wood posing a hazard that could entangle the skin.

Plan of Correction: The nails will be repaired to ensure they are flush to the wood.
