Comments:
An unannounced renewal inspection was initiated and concluded on May 3, 2023 to determine compliance with the requirements for Voluntary Registration. The licensing inspector was on site from approximately 9:40am until 11:15am. The provider assisted the licensing inspector during the inspection. There was one child in care, three years of age with one caregiver present. The licensing inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, and medication. The inspector reviewed two children?s records, one provider?s record.
Information gathered during the inspection determines non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review and interview, the provider failed to obtain a copy of the results of a search of the central registry every five years.
Evidence:

Description:
Based on review of two children?s records, the provider did not maintain the required information in the record for each child enrolled in care. Evidence: The record for Child #1 (first date of attendance 10/19/19) and Child #2 (first date of attendance 10/19/19) did not contain a name, address, and telephone number of one or more persons designated by the parent or parents to be called in case of emergency when a parent cannot be reached during the hours the child is in care.

Description:
Based on review of documentation and observations, the provider does not meet all the criteria on the health and safety checklist at the time of the renewal inspection.
Evidence: 1. The record for Child #4 (first date of attendance 09/12/2022) did not contain a copy of a physical examination results obtained within 30 days after enrollment.
2. The provider stated the practice emergency evacuation drills monthly have not been completed on a consistent schedule. The documented drill log indicated the last evacuation drill was completed on 10/22/2022.
3. Four electrical outlets located in the areas where Child #1 was present did not contain electrical outlets that were child-proof.
