Comments:
An unannounced renewal inspection was conducted on 4/19/23. At the time of entrance, 1 child(grandchild) was in care with the provider. The record sample size consisted of 4 children's records and the provider's record and household member's record. There have been no injuries since the last inspection and no medications are being administered at this time. Areas of noncompliance are noted in the violation narrative and were discussed with the provider during the exit meeting at the conclusion of the inspection.
Bases on record review and interview it was determined the provider did not ensure that all requirements on the health and safety checklist were met. Evidence: 1. There were no records on emergency drills being practiced from July 2022 until April 2023. 2. The records for child #1, #2, and #3 did not contain: -Name, address and phone number of an emergency contact -Names of person authorized to pick up -Emergency medical authorization -Liability insurance disclosure -Medication administration policy -Proof of identity -Physical exam 3. Child #4's record did not contain: -Names of person authorized to pick up -Emergency medical authorization -Proof of identity -Physical exam 4. Provider confirmed that this information was not available during this inspection.
