A complaint was initiated with the local department of social services and law enforcement to investigate an allegation regarding appropriate supervision of children. Interviews were conducted, documentation was reviewed and observations were conducted. It was determined that the complaint was not valid due to insufficient corroborating evidence to substantiate the allegations. There were, however, two violations issued as a result of the inspection. Based on review of three staff records, and an interview with the owner/director, it was determined that not all sworn statement/affirmation forms were repeated every five years as required. Evidence: the most recent sworn statement on file for Staff #2 was dated 10/1/2012 and for Staff #3 dated 3/6/2009.

Based on review of three staff records, and an interview with the owner/director, all staff did not obtain an Office of Background Investigations (OBI) fingerprinting-based criminal history record determination letter prior to September 30, 2018. Evidence: staff #1, staff #2 and staff #3 did not have an OBI fingerprinting determination letter in their file.
